Junior Firefighter Camp Registration 📋🔥🚒

Please read the waiver, medical authorization, photo/media release, code of conduct, and acknowledgment terms below carefully before continuing.

⚠️ Assumption of Risk & Release of Liability
I, the undersigned parent/guardian of the above-named participant, understand that participation in the Junior Firefighter Camp hosted by the Town of Burgaw Fire Department involves inherent risks, including but not limited to:

Physical activity (running, lifting, water activities, drills)
Use of fire service tools and equipment (under supervision)
Slips, trips, falls, and environmental conditions
Exposure to heat, water, and outdoor elements
I acknowledge that these activities may result in injury, illness, or property damage.

I voluntarily assume all risks associated with participation and agree to release, waive, discharge, and hold harmless the Town of Burgaw, the Burgaw Fire Department, its officers, employees, volunteers, and agents from any and all liability, claims, demands, or causes of action arising out of or related to participation in this program.

🚑 Medical Authorization & Emergency Care Consent
I certify that my child is physically able to participate in camp activities.

I authorize the Town of Burgaw Fire Department personnel to obtain emergency medical treatment for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.

📸 Photo & Media Release
I grant permission for the Town of Burgaw and the Burgaw Fire Department to photograph and/or video record my child during camp activities.

I understand that these images may be used for:

Social media (e.g., Facebook, Instagram)
Department/public education materials
Newsletters, websites, and promotional content
I waive any right to compensation or ownership of these images.

 

📋 Code of Conduct & Participation Expectations
I understand that my child is expected to:

Follow all instructions from staff and firefighters
Act respectfully toward staff and other participants
Use equipment safely and appropriately
I understand that failure to follow rules may result in removal from the program without refund.

⚖️ Acknowledgment & Agreement
I have read and fully understand this waiver and release of liability. I understand that I am giving up certain legal rights on behalf of myself and my child.
